SPONSORSHIP AGREEMENT
This Sponsorship Agreement (“Agreement”) is entered into on April 6, 2026, by and between Teri McElrath (“Organizer”) and ________________________________ (“Sponsor”).

Organizer is hosting the above event (“Event”). Sponsor agrees to participate as an Event sponsor under the terms outlined in this Agreement.

• Opportunity to include branded materials or promotional items in attendee swag bags
• Sponsor recognition during the Event
Silver Sponsor – Breakout Session Sponsor – $__350______
• Recognition as the official Breakout Session Sponsor
• Verbal acknowledgment before the breakout session begins
• Opportunity to include branded materials in attendee swag bags
Gold Sponsor – Refreshment Bar Sponsor – $__500______
• Recognition as the official Refreshment Bar Sponsor
• Verbal acknowledgment during the Event
• Opportunity to display promotional materials near the refreshment area
• Opportunity to include branded materials in attendee swag bags
PAYMENT TERMS
Full sponsorship payment is due no later than March 20, 2026, unless otherwise agreed in writing.
NO REFUND POLICY
All sponsorship payments are non-refundable. If the Event is cancelled by the Organizer, the Sponsor will receive a full refund of the sponsorship amount paid.
SPONSOR MATERIALS
Swag bag materials must be received no later than March 20, 2026. Email brand logos to tmcelrath@normalizemenopausehealth.com

Organizer is not responsible for materials received after the deadline.
LIABILITY
Sponsor participates at its own risk. Organizer shall not be liable for any indirect or consequential damages related to the Event.
GOVERNING LAW
This Agreement shall be governed by the laws of the State of Illinois.
ENTIRE AGREEMENT
This document constitutes the entire agreement between the parties regarding Event sponsorship and supersedes all prior discussions.
